TheCTimagesdemonstratedistinctanomaliesthathavebeendescribedasoftenoccurringtogether,althoughbotharerelativelyrare.First,thereisanelongatedhorizontalpetrouscarotidcanalandabsenceoftheverticalpetroussegment.Thecarotidarterycourseisthroughthemiddleearasitentersfrombelowatanenlargedtympaniccanal.Thereisnobonycoveringtotheintratympanicsegment,and,inthiscase,thevesselabutsthetympanicmembraneandtheossicularchain.Laterally,thearteryhasastenoticappearance,typicalforthisentity.

Second,thereisabsenceoftheleftforamenspinosumthroughwhichthemiddlemeningealarteryenterstheextraduralspaceintracranialasitarisesfromthemandibularbranchofthemaxillaryartery.Absenceorhypoplasiaoftheforamenspinosumisadirectassociationwiththeassociatedseconddiagnosis.FurtherevaluationoftheCTimagesrevealsanapparentsoft-tissueprominence,asymmetricfromtherightside,alongtheexpectedcourseofthetympanicfacialnerve.Infrequently,theremaybesubtleerosionofthecochlearpromontory,whichmaycontributetosensineuralhearingloss.
